I would like to say that I am also having trouble getting MySQL to work through a SSH Tunnel.

I've taken your LocalPortForward application, Installed the MySQL-Connector-Net-5.06, added it as a refrence made the forwarding port 3306 and just added a few lines.

At the top
Using MySql.Data.MySqlClient;

Attached to a button event
            String sQuery = "Select id from users_sequence;";
            MySqlConnection cn = new MySqlConnection("Database=itsfeedback;Uid=UserID;Password=Password;pooling=false");
            DataTable data = new DataTable();
            MySqlDataAdapter da = new MySqlDataAdapter(sQuery, cn);
            MySqlCommandBuilder cb = new MySqlCommandBuilder(da);

it will stop on the Fill

P.S. Replace the User-ID and Password with appropriate values

The table users_sequence is a single table with one field "id" that is of type Int(11) and manually assigned the value 30.

If you want source code i'll be glad to share what i've written
