I am eveluating version 3.1.89.

What I did:
1. Call InstallIcon to install my icon to CBFS.
2. Reboot my computer.
3. Call SetIcon after AddMountingPoint to apply my icon on the virtual drive.

The icon appeared on the drive, but it overlaid on the default icon instead of replacing it.

The dimensions in my icon file are: 256x256, 48x48, 32x32, 24x24, 16x16.

Is this by design? Or what else I should do to replace it? Thanks.

Custom icons is implemented through the icon overlay mechanism and restricted to 25 percentages of original icon size (except 16x16 icons). Overlay icons maps as follow:
Core shell code (for example, a shortcut) 10x10 (for 16x16), 16x16 (for 32x32), 24x24 (for 48x48), 128x128 (for 256x256).

So there's no way to replace the icon instead of overlaying it? Or can we replace the icon completely?

No, you can't replace the icon. Maybe it's possible via some very tricky shell extension, but CBFS doesn't support this.
